What is Unum Dental HMO Plan? +

Unum Dental HMO Plan is a Knox-Keene licensed Dental Health Maintenance Organization, offering a variety of prepaid and discount plans throughout California.

What charges will I pay other than the premium? +

For dental treatment, you will pay directly to your assigned Unum Dental HMO Plan contracted provider the co-payments listed on the Benefits Schedule for the plan that you choose. For procedures not covered by your plan, or for treatment by a non Unum Dental HMO Plan provider, including specialists, you will be charged the usual and customary fees of those dentists.

May my family be covered?+

Coverage options for groups are subject to the terms of the group’s Evidence of Coverage. If you have an individual plan, you may extend coverage to your spouse, your unmarried children until age 26, and mentally or physically challenged dependents living at home. Where do I receive my dental care? +

You may receive dental care from the assigned dental office you choose when you submit your application. You can always choose a different office throughout the term of your membership. To do so, please contact a Customer Service Representative at 1-800-937-3400.

Do I have specialty benefits coverage? +

Most of Unum Dental HMO Plan’s plans include specialty coverage. Please refer to your Evidence of Coverage or contact a Customer Service Representative at 1-800-937-3400, for more assistance.

Does Unum Dental HMO Plan cover braces? +

Many Unum Dental HMO Plans provide orthodontic coverage. Please refer to your Evidence of Coverage or contact a Customer Service Representative at 1-800-937-3400, for more assistance.

What if I have an emergency while I'm away from home? +

If there is no Unum Dental HMO Plan provider within 30 road miles, you may seek emergency treatment at any dental office. Unum Dental HMO Plan will reimburse you up to $50 upon presentation of a receipt or statement showing the emergency treatment received by you or your covered dependent. How may I enroll? +

Unum Dental HMO plans are available for groups through employers. Contact your employer or your group insurance agent or broker for more information. If you require further assistance, you may contact a Customer Service Representative at 1-800-937-3400.
